Cook County Public Health and Human Services releases a Request for Proposals for the 2027 Public Health Fund

Cook County Public Health and Human Services (PHHS) issues a Request for Proposals (RFP) through the Public Health Fund for programs to meet the following needs for Cook County residents, as prioritized through the most recent Community Health Assessment:
- Affordable housing
- Mental health resources and support for well-being
- Affordable, quality childcare
- Transportation options
- Affordable, healthy foods
- Physical health services
Both new applicants and former grantees are invited to apply as are both established programs and new initiatives. Applicants must be a 501c3 organization or have a 501c3 fiscal sponsor to be eligible.
There will be a virtual information session on Wednesday, July 22nd at noon to provide prospective applicants with information on the application process and a chance for applicants to ask questions. The meeting link will be posted on the Cook County PHHS website or by emailing [email protected]. A recording of the information session will also be posted on this site for those who are unable to attend, along with copies of the application packet, budget template, and a link to the online application.
