FLU SHOTS will be offered at the following OUTREACH LOCATIONS

• Thurs. Sept. 27 Sawtooth Mountain Clinic NOTE: 2 Times! 11:30am-1:00pm & 4:30-6:00p Lower Level
• Thurs. Oct. 11 Sawtooth Mountain Clinic NOTE: 2 Times! 8:00am-9:30am & 4:30-6:00p Lower Level
• Tues. Oct. 16 Cook County Coop 11:30am - 12:30pm
• Thurs Oct. 18 Sawtooth Mountain Clinic 11:30am-1:00pm & 4:30-6:00p Lower Level
• Tues Oct. 23   Note Two Times and Locations!          Birch Grove Clinic 10:00am - 11:00am  and at   Arrowhead Electric 3:15pm - 4:30pm

If you would like us to bill your insurance, please bring your information and card with you, otherwise there will be a $28.00 charge payable by cash or check only.
