Sawtooth Mountain Clinic Sliding Fee Scale (SFS) season ends on May 31st. This means that to have continuous coverage, anyone on SFS needs to turn in a new application with proof of income by May 31st.  A copy of the SFS application packet in both English and Spanish can be found below.

While May 31st is the deadline for renewing SFS for continuous coverage, applications can be submitted at any time throughout the year. Applications must be turned in the day of a clinic appointment in order to apply SFS to that visit, SFS cannot be applied retroactively. Proof of income needs to be submitted within 30 days.
