Draft 2023-2027 Community Health Assessment Released, Meeting Set for Community Discussion on June 14

Every five years Cook County Public Health staff is tasked with facilitating a process to assess the health and well-being of the Cook County community, and to engage the community in prioritizing areas of health to improve during the next five years. A planning group continues to chart a course toward accomplishing the shared goals associated with improving each priority health area. The Community Health Assessment and Community Health Improvement Planning processes are required of local public health agencies under Minnesota State Statute.

Cook County has completed a draft of its 2023-2027 Community Health Assessment (CHA) and is hosting an interactive, hybrid meeting on Wednesday, June 14 from 9:00 a.m. to noon at the Cook County Community Center to discuss the draft CHA and to give members of the public opportunities to engage with local health data more deeply. There is also a virtual option to attend the session on June 14. Pre-registration for the event is preferred to ensure adequate facilitation of both the online and in-person.

There will be a second follow-up hybrid meeting on July 13 with an in-person component at the Community Center from 1:00 p.m. to 4:00 p.m., with a focus on moving from the community health data to developing shared goals and priority health areas for the next five years. These priority health areas will guide the work of our local public health department for the next five years.
