From Cook County Public Health and Human Services - March 1, 2021

Officials from the Cook County Emergency Operations Center and Public Health announced today that Sawtooth Mountain Clinic patients age 60-64 with certain underlying medical conditions or social stressors are now eligible to receive the COVID-19 vaccine.

To be eligible for vaccination this Thursday, March 4 at the Cook County Community Center you must be: 

• Age 65 or older or
• A patient of the Sawtooth Mountain Clinic age 60-64 who lives with either an underlying health condition or a social stressor that puts you at higher risk for COVID-19

Registration is required for this and all future vaccine events. Visit the Cook County COVID-19 hub site at www.cookcountycovid19.org for more information and a link to complete the online registration. To register, individuals will need to complete a pre-registration survey and provide:

• Name, address, phone number and date of birth 
• A valid email address

Assistance is available for those who do not have internet access or need additional help signing up for an appointment. Call 218-387-3668 or email [email protected] or reach out to a trusted friend, family member or service provider.

Additionally, some 500 individuals will be receiving their second dose of the COVID-19 vaccine the week of March 1.  If you received your first dose at an event on either February 3 or February 4, please see your vaccine record card for the date and time of your second dose appointment. You may not get a reminder call or text due to an issue with the Clinic’s appointment reminder system.  If you have questions about your second dose appointment, please call the Sawtooth Mountain Clinic at 218-877-8273.

Cook County Public Health will continue to post information about who is currently eligible to register for a vaccine as well as how people in that group can sign up.  Registration for weekly vaccine events will open each Monday at 8 am.
