Cook County Public Health and Human Services Releases 2023-2027 Community Health Assessment, Alongside Priority Health Areas

Cook County Public Health and Human Services proudly releases the 2023-2027 Cook County Community Health Assessment Report, alongside six updated priority health areas that represent a shared understanding of health-related needs for the community among the department and the community at large over the next five-year period. These updated priorities include access to:


  • Affordable housing

  • Mental health resources and support for well-being

  • Affordable, quality childcare

  • Transportation options

  • Affordable, healthy foods

  • Physical health services


Over the past year, a group of over 35 community members came together through a series of three community meetings to review relevant health data from local, regional, and state level reports and data sets. The groups shared a vision for a Cook County where everyone deserved the opportunity to be the healthiest version of themselves.  

During these meetings, there were multiple conversations about the various health outcomes for the community, as well as the various causes and contributors to challenges with health. Ultimately participants voted on their top three areas. Conversations continued and will continue throughout this five-year cycle about how community agencies can work to address these various health priority areas. 

The Community Health Assessment is required of local public health agencies every five years under Minnesota State Statute. This report forms the basis of the Community Health Improvement Plan, which will be under development over the course of the five year period of 2023-2027.
